Case Management Services

Case Management services assist an individual and their support network to identify, select, obtain and coordinate paid and natural supports as needed to enhance an individual's independence, integration and productivity in their community. Case managers may or may not work for licensed providers, but are expected to function in ways prescribed by the Kansas Department of Social and Rehabilitation Services in the performance of service coordination duties. Case Managers must be registered with the SRS Division of Health Care Policy, Community Supports and Services, be employed by an organization that subcontracts targeted case management services with the Reno County CDDO, or is an individual, independent case manager that subcontracts targeted case management services with the Reno County CDDO, and obtains/maintains a license to provide targeted case management services current license with the Kansas SRS Division of Health Care Policy, Community Supports and Services.
